Transportation Assistance Qualification

Must be under 200% of the Federal Poverty Level and up to date on eligibility:

Your case manager will refer you for one of the following:

• 31 day bus pass – Can be picked up from the 1st business day of the month until distribution day.
• Gas cards – Must track your mileage for medical related appointments. You will pick up gas cards on distribution day. Cards are retroactive.
