Clinical Services Manager, R.N.
Reports To: Director of Medical Services
Status: Full-time, nonexempt
Beginning Wage: DOE
POSITION PURPOSE
Conduct new client intakes, perform triage on unscheduled clients and oversee nursing staff’s daily operations. Provides direct supervision for LVN’s, MA’s, front office staff and Chronic Care Coordinators.
PRINCIPAL RESPONSIBILITIES
-
As part of a Case Management Team, develop a comprehensive, individualized service plan for continuity of care;
Oversee pharmacy vouchers program and agency AIDS Drug Assistance programs;
-
Conduct intakes on new clinic patients;
-
Record, monitor and coordinate client medication and drug treatment programs;
Coordinate and supervise patient flow in the clinic.

Perform triage and clinical assessments for scheduled and unscheduled clients;
-
Prepare patients for physician appointments and follow up as necessary;
-
Continuous Quality Improvement (CQI) Coordinator
-
Coordinates Guatemala medication program;
-
Supervise LVN’s, MA’s front office staff and chronic care coordinators working in clinic. Supervision includes approving work schedules, setting goals, performance evaluations, and coaching.
TRAINING, EXPERIENCE, ABILITIES
-
RN licensure in the State of California
-
Minimum of five years experience in a clinic, physician’s office, medical facility, public health department, or similar facility;
-
Prior supervisory experience required;
-
Experience in laboratory surroundings to include patient instruction, triage and specimen requirements;
-
Knowledge of HIV/AIDS care and treatment and sensitivity to issues and populations impacted by the disease;
-
Medical-Surgical Background;
-
PHN preferred.
-
Bilingual/bicultural preferred.

COMMUNITY HEALTH EDUCATOR
Reports To: Director of Social Services
Status: Temporary, Hourly, Non-exempt; No Benefits
Beginning Wage: $15.00 / Hour
POSITION PURPOSE
Provide HIV/AIDS prevention education to the general public and special populations. Keep abreast of issues pertaining to HIV/AIDS concerns, including medical, social, and current affairs. Provide educational services to the community for prevention purposes and provide a variety of education and training programs as assigned.
PRINCIPAL RESPONSIBILITIES
-
Assist in managing the Positively Speaking program;
Provide HIV 101 presentation support to the schools;
-
Coordinate programs / presentations on HIV / AIDS prevention education to new and existing contacts;
-
Provide ATS counseling. Maintain certification as required by the state;
-
Ensure all goals and objectives are met, as required by the Director of Social Services, including program statistics, and periodic reports;
-
Communicate information in a manner consistent with agency training standards;
-
All other relevant duties as assigned.
TRAINING, EXPERIENCE, ABILITIES
-
Experience in medical or health-related fields including: social work, counseling, nursing, health education;
-
Two years experience in the field of AIDS education preferred;
-
Experience in working with the substance abuse community, minority groups, health care workers and youth;
-
Experience working under grant funding, implementing same and reporting;
-
Professional demeanor in presentations;
-
Previous public speaking/presentation experience required;

Strong writing and verbal skills preferred;

Bilingual/bicultural preferred.

FULL-TIME CHRONIC CARE COORDINATOR
Reports To: Director of Case Management
Beginning Wage: $15.00 / Hour DOE
Status: Hourly; Full -time, Non-exempt
POSITION PURPOSE
To coordinate the delivery of services to an assigned client population of persons with HIV infection in a manner consistent with the Policies and Procedures of D.A.P. and related program protocols. Too triage clients and participate as a member of the Case Management Team. Serve as a representative and act as a resource and referral source for clients, making recommendations concerning their social and practical needs.
PRINCIPAL RESPONSIBILITIES
-
Link clients to appropriate referral sources;
-
Assess the client's mental, social, financial, and functional status when appropriate;
-
Serve as a member on the Case Management Team and participate in case conferences;
-
Conduct monthly intake orientation meetings for new clients;
-
Act as liaison between client and community social services, and act as clients advocate, where necessary and appropriate;
-
Submit referrals for insurance needs, dental care, mental health counseling, transportation, housing, food, legal and/or other D.A.P./community programs for self managed clients as needed;
-
Provide crisis intervention when necessary and appropriate;
-
Assist the client and service providers in problem solving as appropriate;
-
Cross train in other positions as may be required or assigned;
-
Maintain accurate records of all client interactions in client services data base in a timely manner;
-
Work w/volunteers to maintain updated records in self managed client files;
-
Deactivate client’s files after one year of inactivity
-
All other relevant duties as assigned.
TRAINING, EXPERIENCE, ABILITIES
-
BA in psychology, medical or social services, or related fields or equivalent work experience in a similar position;
-
Demonstrated familiarity with standard procedures of a comprehensive case management system;
-
Flexible hours and/or shift variation may be required;
-
Bilingual/bicultural preferred.
