Becoming A Client

Intake And Update Required Documents



Intake Class for new clients – Every Wednesday at 2pm

You must call Ray Robertson at 760-323-2118 x295 or email for an intake appointment. Read about the entire process here.

Required items needed to attend the Intake Class

  • Completed Intake Packet- Download here (Spanish version)
  • Photo ID - 1 of the following:
    • Driver's license
    • State ID
    • Immigration Card OR
    • U.S. Passport
  • Proof You've Lived in Riverside or San Bernardino County for 30 days
    2 of the following, indicating name and address:
    • 30 days current utility bills
    • current rental or lease agreement showing at least 30 days
    • California Driver's License/California Identification Card with current address
    • Voter Registration Card
    • Signed affidavit of residency from person you are residing with
  • Proof of HIV Diagnosis
    • Diagnosis information form or letter completed by a physician, physician's assistant, or nurse practitioner indicating that the individual is HIV PositiveOR
    • A HIV positive laboratory result that includes the individual's name
  • Proof of Insurance (Only if you are planning on becoming a patient of the Health Center)
  • Income Verification - 1 of the following:
    • 3 current pay stubs
    • 3 months current bank statements
    • SSA, SSI, or SSDI Award letter
    • MISP/CMSP/Medi-Cal Award letter
    • Letter from other form of Government assistance
    • Affidavit Letter of Support

Update Class for current clients – Mondays at 3 PM, Tuesdays and Thursdays at 10 AM

The Health Resources and Services Administration (HRSA) is the federal agency that provides for Ryan White grant funding.  HRSA now requires that all persons receiving Ryan White-funded services be screened for eligibility through an update process every six (6) months.

You must call Chuck Bentz at 760-323-2118 x709 or email to reserve a space in the update workshop. Leave your first name, phone number, and the date you would like to attend.  

Required items for Update

  • Labs
    If you receive medical care with a provider outside of D.A.P’s clinic, bring a copy of your most recent labs showing CD4 & Viral Load and a copy of your TB Test. 
     
  • Proof You've Lived in Riverside or San Bernardino County for 30 days
    2 of the following, indicating name and address:

American Sign Language and Language Translation services available. Please request 2 business days in advance. Contact your case manager.
